Mankato health officials are asking parents bringing their children to the H1N1 vaccination clinic Saturday to fill out the following form. The free clinic is for children age 6 months to five years old.
The vaccinations will be 10 a.m. to 3 p.m. Saturday at the ISJ Specialty Clinic, located at 1015 Marsh St. Mankato. Use entrance 2 on the north side of the medical center.
